Medical records are kept strictly confidential and will not be disclosed without your written permission.

Patients requesting a copy of their medical records must sign an authorization form. To expedite your visit, please feel free to print and complete the downloadable authorization form in advance of your office visit.

The sign-up process is simple; all you need to do is call our office, share your email address, and ask for the portal registration to be sent via email.

To release a copy of your medical records: An ROI form must be completed, and proof of identification must be provided. The ROI form must be delivered in person or mailed with an original signature. Your form will not be accepted via email or digitally.
